Heerenveen went into the clash on the back of three straight victories and in the fourth minute they were ahead again. Henk Veerman fired a shot past Martin Paes after being set up by Rodney Kongolo.
It was nearly 2-0 shortly afterwards but Mitchell van Bergen struck the bar with a good strike.
As the half wore on, Utrecht improved, and eventually, Gyrano Kerk equalised with an excellent fierce strike.
Henk Veerman was forced off through injury, meaning young striker Rein Smit came on for the second half. Twenty minutes after coming on, Smit was sent off for his second yellow card.
From that point, Utrecht poured forward in search of the winner. Eljero Elia went close from range, before Mimoun Mahi hit the post late on. Heerenveen held on for the point, which takes them top above Ajax.
