The home side started strongly, and took the lead on twenty minutes when captain Nick Viergever got in front of Joshua Brenet and headed a corner into the net.
However PSV equalised on 36 minutes, with Memphis Depay netting with a stunning free kick, which the youngster rifled past Alvarado in the AZ goal from just outside the box.
Just before the hour mark AZ were back in front, as Aron Johannsson hit a brilliant half volley over substitute keeper Tyton, and into the net.
PSV striker Tim Matavz then had a good chance to instantly equalise, but he saw his header tipped over the bar by Alvarado.
The introduction of Adam Maher, making his first return to Alkmaar since moving to PSV in the summer, and youngster Zakaria Bakkali failed to inspire the away side, and the match finished 2-1 to the hosts.
PSV stay temporarily top of the table, but that could change when PEC Zwolle meet NAC breda later, while AZ Alkmaar climb into the top three, after an impressive performance.
