The home side started the game brightly, but the post denied Felipe Gutierrez before the follow up was cleared off the line by Derrik Luckassen.
Hakim Ziyech and Zakaria El Azzouzi also came close to breaking the deadlock, but AZ grew into the game as the first half progressed and Levi Garcia was unlucky to see his effort crash off the bar after a fantastic run.
Six minutes into the second half AZ did take the lead and it was Vincent Janssen who fired a low strike into the bottom corner for his 20th goal of the season. Janssen, who received his first national team call-up this week, is now top scorer in the Eredivisie.
After the hour mark the hosts were reduced to ten men with El Azzouzi seeing a straight red for an elbow on Ridgeciano Haps. However, the teams were shortly evened up when Stijn Wuytens was shown his second yellow for a trip on Ziyech.
Twente then introduced German defender Stefan Thesker, who was making his comeback after having a tumor removed, from the bench. Moments later the defender rose high to head in a free kick and level the scores.
Parity was short lived as AZ went straight up the other end and Thom Haye hammered a strike into the top corner to restore the visitors lead.
Twente pushed for a late equaliser and after Ziyech hit the crossbar, Thesker netted his second header in the first minute of injury time to seal a point.
Twente rise to tenth, while AZ remain fourth.
